ABD'de 1998 yılından beri yaşanan büyük kimyasal kazaların bir daha yaşanmaması maksadıyla ders çıkarmak için araştırıldığı U.S. Chemical Safety Board platformu ülkemiz için de çok kıymetlidir. Bu sitede mercek altına alınan araştırılması tamamlanmış 80 adet ve araştırılması devam etmekte olan 7 adet kimyasal kaza olayı bulunmaktadır.

Atasözümüz "Bir musibet bin nasihatten evladır" der. Elbette asıl olan musibet başa gelmeden ilim yolu ile bu kazaları oluşturabilecek tehlikeleri bilmek ve anlamak, kazaları oluşturabilecek riskleri ölçebilmek, sonuçta gerekli tedbirleri alarak ve riskleri minimize ederek kazaların oluşmasına fırsat vermemektir. Tecrübe yolu ile öğrenme biçimi ise bazen hayat kaybına da mal olan çok pahalı bir öğrenme biçimidir.

En acı olanı ise tecrübelerden bile ders çıkaramamaktır. Ülkemizdeki bu tip kazalar da tarafımızdan mercek altına alınmalı, aynı şekilde raporlanmalı ve her kesim için alınacak dersler çıkarılmalıdır.

05.01.2012 tarihinde yayınlanmış olan rapor, aynı fabrikada demir tozunun 6 aylık zaman diliminde yol açtığı 3 adet yangın-patlama olayını incelemektedir. Demir tozunun ilginç yanma-patlama davranışını gösteren bu çalışma; NFPA 484'de bile revizyon yapılmasını gerektirecek yeni bilgiler içermektedir. Kimyasal Kaza 70 kodu ile alt tarafa koyulmuştur.
